1. Enter your user name. 

Unless you have changed your user name, use the default version. It is the first letter in your first name, the first three letters in your last name, and your date of birth (mmddyy format).

Example. For Jane Smith, born on July 4,1976:
jsmi070476

2. Enter your password. 

Unless you have changed it, your password is your birth date (in mmddyy format).

Example. For July 4, 1976: 070476
